By submitting a request for an essential worker COVID-19 at-home collection kit, I am attesting that I qualify as an "essential worker" as defined by the California Department of Managed Health Care (DMHC) and that I am eligible to rder the kit based on my Health Net health plan coverage.
IMPORTANT: When completing your request on the following screens, please be sure to select "Health Net California HMO/PPO Essential Worker" under the Insurance section.
How it works
1. Provide your information and create your Pixel by Labcorp account
2. Receive your sample collection kit via FedEx. Most kits will arrive in 1-2 business days; there may be a delay if your order is placed on a weekend or around a holiday.
3. Register your kit, collect your sample, and send it back to our world-class lab for testing
4. Access your results online
